Apple TV constantly freezes the video (continuing to play audio) while watching shows and movies through my PS4. Stays frozen for about 10 seconds. Then resumes as normal. Repeating intermittently every 5-10 minutes or more.

Based on your post it would appear the there is a streaming issue using Apple TV+ on a PS4.
The first thing we would recommend would be to remove the app from the PS4, power cycle the console, and then install the app again.
Be sure the console is connected to a good quality high speed internet connection.
