Hello. I would like to ask you if I have to do a port forwarding to be able to access the homekit accessories outside the LAN network. and possibly which ones? thank you

No port forwarding is required, but you do need a device that acts as a home hub. See the linked article, Set up your HomePod, HomePod mini, Apple TV, or iPad as a home hub - Apple Support.

I can see the status of my devices when I'm at home, on another network, say at work, or on mobile Internet. It should be same for you. If you haven't already done so I would reboot your router and then reboot the Apple TV.

I already have an apple tv 4. Until a couple of weeks ago everything worked fine. Then if not connected to the lan all devices are offline, even if I receive push notifications
