Serious trouble with Apple Pencil (gen1) behavior

I've tried changing the nibs, switching pens (since I have two), charging the pens (both) and resetting the connection AND my IPad, but the issue keeps returning. It seems to just pick up random input and make... Lines? I took a video of it, but it's too large to upload here, so I've attached some screenshots.

This happened with me hovering the pencil a few millimeters above the screen and moving it up and down. When the pencil connects with the screen I have the same results, except parallel with the angle I'm holding the pencil at.
