Your cellular provider is the one that sets up Personal Hotspot. It has to be activated on their end first. Then see this, How to set up a Personal Hotspot on your iPhone or iPad - Apple Support and this, If Personal Hotspot is not working on your iPhone or iPad (Wi-Fi + Cellular) - Apple Support
