A. Was this Purchased from Apple?:
If this was purchased from Apple, and you have coverage, contact Apple Support about getting this serviced. If this was purchased as a used device, then the camera may not be official, and a flawed item.
B. Contact Apple Support:
Contact Apple Support, even if this is not covered, and ask the Apple Rep on the phone to look into this. If desired, perform a screensharing session with them, so that they can see this firsthand from your perspective.

C. Restore form a Backup:
Have a backup of your iPad prior to this update? If you are not concerned with losing data, then restore your iPad from a backup that was created prior to this occurrence. If you have no backup, then take this as a learning experience knowing to always backup your iPad, every to often. Important: Prior to restoring, backup your iPhone, incase anything goes wrong with the restoration.
