How to get My Health app backed up on iCloud

I’m getting a new phone and it’s imperative that My Health app transfers to the new phone. How do I get it into iCloud?

It is already synced to iCloud if you have enabled it under iCloud on your device

I do have it enabled but I wasn’t sure it included Medical ID or just the activity/heart rate, etc, data.

I am looking on my iPhone because my computer is out of commission and you can’t see everything on the iPhone.
