iPhone XR not ringing when calls from my contacts come in and not showing calls in recent calls list

For the past couple months I've noticed some calls from my contacts list wouldn't ring and I'd either miss them or they would go straight to voicemail. Yesterday I had a Doctor call, who is in my contacts, and it never showed up as a call missed just went straight to voicemail. I've checked all settings so many times that I'm numb. I do not use the "do not disturb" setting and it's not on. I'm running the 14.7.1 iOS. I've even turned off the "silence unknown callers" tab and the contacts are still not coming through EXCEPT the other day for the first time and the only difference was I was away from my house, I was sitting in a parking lot when the doctor's office called me. Today a couple "unknown calls" came through but they don't show in the recent list as either ALL or MISSED. Any assistance would be greatly appreciated. I'm exhausted.

Have you tried a restart?

Otherwise, the standard steps for not ringing are as follows

Is the side silent mute button on? Does any red show? Slide it back and fourth.
How about the volume buttons? Is the volume very low? Move the buttons.
You said that you've already checked, but: Is Do Not Disturb on? Go to Settings > Do Not Disturb. Is it turned on? Check the times.
Go to Settings > Sounds & Haptics. Check the Ringer and Alerts slider. Try changing the sound pattern for the type of alert.
Go to Settings > Notifications. Is the app listed?
Do you have an Apple watch? Many of the notifications will go to the watch instead of the phone.

Note: you can send text messages to yourself for testing purposes.

Yes all other sounds work fine. The mute button on the side is off (no red) and the volume is maxed out. The DO NOT DISTURB is definitely not on. I've clicked it on and then off again just in case. I do not have an Apple watch just this iPhone. I have not tried a restart yet besides a basic shutdown. I'm concerned I will lose all my contacts and photos if I do a complete factory reset. Is that a possibility ? Oh and I have changed the ringer tone and tested all of them and they worked fine so I reset the ringer back to default. Thank you for the suggestions. I really do appreciate any help I can get.

Yes, a factory reset would be the next step. As indicated, perform a backup first so that your data can be restored afterward:

Restore your iPhone, iPad, or iPod to factory settings
