I’m skeptical about those yearly cleaning recommendations, outside of computers used in very dusty environments.
Sometimes getting hot is normal when performing heavy computations or when rendering complex graphics; when running high-resolution video games, for instance.
Start a service call with Apple about the overheating, if it concerns you.
If you want somebody to perform the equivalent of preventative maintenance, you’ll need to discuss that with an Apple Authorized Service Provider, who may be willing to perform that task.
And I’d look to use the MacBook Pro in an area with less dust, as that dust can’t be good for your lungs either.
There are directions for opening the MacBook Pro yourself posted around the ‘net, if you’re willing to acquire the necessary tools, and willing to work on fragile and static-sensitive hardware. And I wouldn’t be looking at the paste replacement absent an indication that the processor was overheating.
And any damage incurred here will not be covered by the Apple warranty.

Opening the Mac on your own could void the warranty, not recommended.
If the Mac was too hot, it would shut itself down. The Intel Macs do get warm, yes.
If you are having "very hot " issues on the 16" MBP , one recommendation is plug he charger into the right side ports, all peripherals, external monitors on the left hand side ports.
