IPad Pro - Rear Camera not saving photo

My newly bought IPad Pro camera is not working. Have issues below:

taking photo does not save anything.
sometimes after restarting. It saves some photo but it turns into a video (not live). A video with actual play button
tapping (x1) on camera makes the whole camera not responding.

note: taking video is working (except (x1)).

Already did:

update latest software
restarted ipad (multiple times)
factory reset ipad (multiple times)
