white grid app icon, Epal, please help!

This icon showed up on my iphone after an update to ios 14.8. I searched and searched, and finally restored my new device, (only had it a week.) and restored to a backup from several weeks ago to avoid this icon. then 3 days after the restore, this icon shows back up. this is getting so maddening, on an android device, i could just run a file manager as root, and delete the pesty icon, and this ios, i have no control, and there is no way to delete the icon. I've looked in shortcuts, and all my apps, I systematically deleted every app, and the icon stays. This is a major bug in ios. I should be able to remove this icon, maybe the new update has a virus. See the image attached. it says Epal. and there is no maching app or shortcut info on my phone. this is an iphone 11 running ios 14.8, and restore did not fix, it just showed back up. please help!
