Otterbox Defender causing choppy voice conversations on iPhone 11 Pro

I purchased an iPhone 11 Pro two years ago and bought an Otterbox Defender case for it. My phone worked great unless I was talking to my daughter, who always had children and dogs in the background. She simply could not hear me speak most of the time, but I heard her clearly. After much troubleshooting and testing I discovered that the Otterbox case was blocking my side of the conversation. I removed the outside rubber shell and the problem went away. Currently, I only use the inner plastic Otterbox case and the screen protector on my phone and it works great, EXCEPT it's hard to reach the buttons on the side of my phone without the outside rubber case and button extenders. Has anyone had similar experiences? Is there a protective case that does not impede voice phone calls?
