Why apps downloaded to my paired iPhone don’t download to Apple Watch with automatic down selected?

It may be downloaded to your watch but I believe you still need to install it. On your iPhone, tap on:

Watch > My Watch

and scroll down to the "AVAILABLE APPS" section and tap on the INSTALL button.
