Old Exchange Account Keeps Returning to Internet Accounts

About every couple days or so I get an “Exchange Account Added” notification for an old email address from a previous job. I have to go to Internet Accounts and manually delete it. I’ve gone through Keychain Access and removed every entry related to the old email address. I’ve also checked that the account has been removed from all my apps and mobile devices, but the Exchange account keeps returning.
