I'm not seeing an option to add my COVID-19 Vaccination record, which I've already got in the Health app, to Wallet. I've connected to my provider (Kaiser) and can view basically my entire record. I'm on iOS 15.1 on iPhone 11.

Your provider needs to have the option to do it, as indicated in the link that you were provided. If they don't have the option, then you cannot add it.
OK, finally figured it out. I went to my provider's website (KP.org) and was able to bring up the QR code I'd read about. Scanned it with the Camera app, tapped the "Health" pop-up notification, and boom, "Add to Health and Wallet" appeared. Done!
I had previously downloaded the records from KP and my COVID-19 vaccination info DID show up in the Health app, but there was no way from there to add the card to Wallet.
tl;dr version: The QR code option is by far the easiest!
