Troubleshooting this:
There is debris beneath your iPhone's iSight camera. So, get it out. To do so, take it to Apple. Start by contacting them.

Screensharing with the Apple Rep
Apple needs to show this for themselves. So, screenshare the screen of your iPhone and then open the Photos app. By screensharing the Apple Rep will see this firsthand from your perspective.

Does it sound different, than other apps you use your iPhone on your Mac?
MacBook Pro camera is blurry and poor quality. Our MacBook air 2015 had far better resolution.
The MacBook air works very well. I am surprised the later model of Apple has such a poor camera. Expensive and less quality.

How long has it been since day one? See about getting this serviced. If this were me, I would have this looked into by Apple. You have a one-year, out-of-the-box, limited-warranty. So, contact Apple Support.
