Cant add vaccine card into apple wallet
I've tried uploading my vaccination card, but it's saying that it cannot be added to my apple wallet at this time. The institution that administered it is a verified facility (UCLA). Any idea what is going on?

Try to see if your state of residence offers a QR code for your COVID vaccination record. The state of Washington offers this type of verification. I added mine to the Health and Wallet apps simply by holding my iPhone camera up to the QR code, provided by Washington state, and it allowed me to add my vaccine verification to the Health and Wallet apps - the whole process took a few seconds.

My wife had a similar symptom; her record would not add to the wallet. The problem was that her name on the vaccine record did not exactly match the spelling that was reported to the state registry.
What was the result when you followed the instructions in the Related Article above your name on this page, that you were reading just before you posted here? Specifically the instructions for adding it to the wallet from the health app:
If you have an existing verifiable vaccination record stored in the Health app
If you previously scanned a QR code, downloaded a file, or connected with your healthcare provider to add your health records on your iPhone or iPod touch, you can add your stored verifiable COVID-19 vaccination record to Wallet.
If you don't see the Add to Wallet option:
Also check that you have iOS 15.1 or newer.
The facility must provide a verifiable document. If the document they provided isn’t recognized as verifiable you need to contact them.
@revianmac, this worked, and, like you said, only took about 2 minutes. Not sure why UCLA's QR code wasn't working but the state of CA link did load into the wallet correctly.

Following that page, I never see the option to add it to my wallet. My wife, who got her vaccines with me, added hers just fine. I have a feeling that the record from my healthcare provider is preventing the option from showing up for the verified record.
Thanks for the response. It's a verrified document as it was able to be added into the health records on my phone, but it's not loading into the apple wallet. And, for clarification, the phone is updated with the most recent ios.
