The Grand Mafia & Apple Rip People Off

How can Apple deny a refund request for in-app purchases that do not work? The developer has done nothing to fix the issues with the app, and Apple has rejected the refund request. What other options do I have? The three in-app purchases were supposed to enhance the game, but the purchases did not work (time-sensitive tokens were never received). The rewards I was supposed to receive glitched, and I have not received them. The developer took-2 days to respond, and when I finally asked for a refund, they went quiet. I reached out to Apple after another couple of days, and they denied the request. I appealed the decision, they denied it again. What options do I have?
