My phone’s screen time shows that I am using Instagram 12+ hours a day but the Instagram app is not downloaded to my phone. The screen time is also not shared across my devices. This is bizarre.

I found that updating my phone software, turning off and then on screen time, and rebooting my phone helped to resolve my issue.
If you are sharing your Apple ID then learn what to do if you think your Apple ID has been compromised.
I do not share my Apple ID and I just changed my password, yet it still shows that the Instagram app is being used.
