Please do as T-Mobile instructs to redeem the offer. See the FAQ “How do I redeem” near the bottom of the page: Get Apple TV+ on us for one year - T-Mobile

Once you have the subscription, then you may access it on any of your devices with the Apple TV app, by signing in to the Apple TV app, through the in-app Settings (sprockets wheel icon), with the Apple ID account that has the subscription.
