Hello,
I have Iphone 13 and the volume during a normal phone call is very loud even on the lowest setting. My ear is aching. I have tried everything from the settings and the side buttons. Is there any working solution besides changing the phone with another brand?

Welcome to the Apple Support Communities. From your post, your iPhone appears to be too loud doing phone calls. We'll do what we can to help.
Try using the accessibility features to see if they help. They are listed in this link under 'Hearing': Get started with accessibility features on iPhone
Also make sure you are turning down the volume while you are on the call as turning down the volume while on the Home screen only turns down volume for the ringer/notifications.
