My iPhone was hacked also my Apple ID was hacked and my email and Facebook I’ve changed passwords how do I know if I’m still hacked

Your phone wasn't hacked, your Apple ID was. Someone found out, knew, or guessed your ID and password. Once logged in as you on their iPhone, iPad or Mac, they could see everything you do. Contacts, emails, messages, etc.
You're lucky they didn't change your account password, or you would have been locked out of your own account. Go to:
Login to your account. Click the Account Settings link. Under the My Devices heading, do you see any that don't belong to you? If so, that's who's in your account who shouldn't be. Click on it. A question is presented asking if you lost, sold or gave away the device. Put it into Lost Mode (activation lock) and then remotely erase it.. That person's device will become a permanently unusable brick. Serves them right. Leave it in your account since deleting the device entry will remove the lock.
Immediately change your password again. Make sure it's something difficult to guess. Make sure 2 Factor ID is on. Assume this person knows your current 2FA answers and change all of them.
