iPad Numbers

Currently working with the form sheet entering values to automatically fill in values in the spreadsheet. The problem i am having is when the values transfer from the form to the sheet, the info transferred into the spreadsheet is a row off.

How do I fix that?

PS: it is a iPad Pro 5th gen Wi-Fi 12.9 inch
