I used to be able to set two alerts for a calendar event, but now there is no longer the option to do so. When I create a new event in my iPhone’s calendar, it will only let me set one reminder, when I used to be able to set two. I don’t see how to fix this in settings.

Whether that option is available for an appointment will depend on what type of cloud/email service you are syncing that calendar with.
iCloud supports it, Exchange, as an example, does not.
