Only by taking a photo. But to add it to the Health app, you're going to need a QR code provided by a state. A pharmacy providing a vaccination is supposed to transmit the vaccination record to the state health dept. Not sure what state you're in (or even if you're in the United States). But at least in the US many states have a means to produce a digital vaccination record that contains a QR code.
