We got this far, and what you are reporting is an extremely-reported issue on these forums. I haven’t come across an ultimate solution to the “start alone hearing issue” per se. This is now an issue with the iPhone itself — no longer the car.
So what to Try:
A. Unmute it:
Is your iPhone on Mute, while on the phone? If so, tap the Mute button so that it is not red-colored. It is the button that looks like a microphone cover.
B. Contact your Carrier:
Contact your carrier -- report back what they say.
C. Verify the volume
If Nothing Above is of Success: Contact Apple Support
