If you have had on offer for an extended three months trial to Apple TV+, then you have to redeem that offer on your eligible device.
For Apple’s offer, you would need to have bought a new iPhone, iPad, iPod touch, Apple TV box, or Mac. This offer has the condition that it has to be redeemed within the first 90 days. To redeem, find the “Enjoy 3 Months Free” redeem button in the Apple TV app on your new eligible Apple device.

On what device? Eligible Apple devices would have the Apple TV app pre-installed.
Once you have redeemed the offer, then the subscription will be available on any of your devices with the Apple TV app, when signed in with the same Apple ID account.
