It is nearly impossible to hack an iPhone. What makes you believe that your iPhone (Notes) have been hacked?

If you believe that your Apple ID has been compromised, then change your Apple ID Password immediately. You can do so by opening Settings ➡️ Apple ID (Your Name at Top) ➡️ Password & Security ➡️ Change Password.
