Siri in iOS 15.4 fails to make phone calls

Immediately after I updated my iPad and iPhone to iOS 15.4, Siri fails to make telephone calls. For example, when saying Hey Siri, call NAME (a name in my contacts) Siri starts the call but does not open the phone and the call fails. It does not happen to all contacts and, in fact, fails on not all phone numbers in a contact - some phone numbers in a contact will work with Siri while others in the same contact fail. This happens on multiplayer iPhones and multiple iPads. Any suggestions?

Great news regarding this issue: iOS and iPadOS 15.5 fixed the bug. Siri can now dial and make calls with * code prefixes.

My problem with Siri placing phone calls is slightly different. Since updating to 15.4.1 on my iPhone, if I tell Siri to call "XYZ in (location), I get the message "Sorry - you'll need to continue in the app." I then get an open phone button with opens up my telephone app but Siri still won't phone out.
