I called apple support. Turns out this is a feature not a bug
in
Settings/Accessibility/Faceid and Attention
there is a toggle
Attention Aware Features
that will turn the volume down and dim the screen when the iPhone detects a face during a phone call or other things
the Apple advisor suggested toggling this off

Go to Settings/Face ID & Passcode and turn off Attention Aware. This feature will lower the volume when you are looking at the phone.
Well, I’m glad you got an official answer from Apple, confirming what I told you 2 days ago in my post on April 1, above:
Go to Settings/Face ID & Passcode and turn off Attention Aware. This feature will lower the volume when you are looking at the phone.
I’m not sure exactly when it started but when I notice it I can reset the volume with the buttons and they work for other things like screenshots
im running 15.4.1 and it is still happening
slso I tried turning off change volume with buttons but if I press the volume buttons the sound slider still goes up and down and the sound level still changes
does it require a reboot to take effect. This is a 13 pro max
Hello needleman_mark,
We want to help go over the situation with volume on the iPhone. Do the buttons work for other features like screenshots or volume? Do you see the value go up and down as expected with each press?
Update your iPhone, iPad, or iPod touch - Apple Support and check if the latest iOS version 15.4.1 makes a difference.
We hope this helps.
Cheers.
I have rebooted the phone multiple times. In fact tbe update pro was includes a reboot of the iphone
the first thing Apple support is going to so is tell me to do a restore as a new phone in case it’s a software problem with some app
Thanks for reaching out again, needleman_mark. If the update didn't help your issue, as it would reboot your phone upon completion of the update, we'd recommend reaching out to Apple Support: Contact Apple Support
Regards.
