How does it work if you get your kids an Apple Watch but they don’t have a phone. Do you have to buy cellular service. And how expensive is that.

Apple Watch Family Setup requires the cellular version of the Apple watch. You don’ have to setup cellular, but without a paired iPhone, the watch is pretty useless. No calls, no messages, no internet access nothing unless connected to a Wi-Fi network. when connected to Wi-Fi Then only Facetime Calls and iMessage calls are possible unless the watch has cellular service.
For the full function of the watch used without an iPhone, a cellular plan is highly recommended. cost will depend on your carrier. Ask them for pricing and plans for Family Setup Apple watches if they support that.
