I wear MFI hearing aids, connected to my iPhone 12, and using the Lively app. When watching YouTubeTV, how can I switch the audio to my phone speaker as I listen?

You should be able to use Airplay to select the audio output while listening to Youtube Tv. Here is a link to help:

It helps you select your audio output from the Control Center.
Adding: Media Accessibility ==> Hearing Devices ==> Audio Routing ==> both Call Audio and Media Auto set to "Automatic",
