How do I keep iPhotos from duplicating photos on Mac mini?

How are you importing photos? And when are the photos duplicating? Are you importing from an iPhone or iPad or a camera?
Are you using any cloud service to sync your photos? My Photo stream? iCloud Photos?
Are you by any chance converting photos to a different format when downloading them from an iPhone or iPad, for example concerting from HEIC to JPEG? This will interfere with the duplicate detection and is a common source for duplicates, also mixing two different methods of importing - for example My Photo Stream or iCloud Photos and manual importing.
