Is there a process that a country’s government can follow in order to have the ECG application active in the country?

There are two parts to the effort. The first involves approval for the EKG/ECG equipment in Watch and particularly its possession and usage by non-medically-trained and non-medically-licensed and non-medically-credentialed personnel as acquired from the applicable medical regulatory organization(s) in the country or region (lobbying legislative representatives, or lobbying the medical committee(s) or medical board(s) or whatever the group(s) are called locally), while the second is then encouraging Apple to add the now-approved feature in that country or region.
