The installation continuously fails. What to do?
I connect my iPhone to my Macbook and it says it needs an upgrade to chat but the upgrade fails continuously.

To my eyes, it implies some update for macOS Monterey is required for the attached / cabled iPhone to computer.
Do I have that right ?

Shut down both devices manually and reboot both devices—then compare your results.
Restart in Safe Mode. This will perform a Disk Repair, clear cache files and only load Apple Software, extensions and fonts. The boot up will be slow and can take some time - Normal and Try Again

All devices are totally up to date.
And when I click on update it downloads then says it's installing then crashes and says the installation failed. This has happened several times over the last two days.
This began as soon as my MacBook came back up from its upgrade. I had upgraded the iPhone just before that. SO yes, both machines were rebooted.
Exactly correct. The MacOS downloads the update and attempts to install it but it then crashes. This has happened at least a dozen times.

secondly, have seen this issue when iOS 15 was first released
may need to give Apple a little more time to sort out the IOS 16 issues for this specific update to work. ❓
