I just installed IOS 16 yesterday, and this problem just showed up.
No matter what volume my phone (iPhone 12 mini) is set at, when I get a phone call and then insert my airpods in my ears before answering, the volume from my ringtone through the airpods starts out painfully loud.
Is this an IOS 16 issue or did installing the OS mess up some of my previous settings?

I've had the same problem and I found the best solution available so far. The ring volume in your AirPods is tied to the volume in your Sounds & Haptics settings. Go to Settings > Sounds & Haptics: Ringtons and Alert Tones.
Mine is set to 100%, which blew out my ears. I turned it down to about 20%. My setting for "Change with Buttons" is off.
I called my phone from another line with my AirPods in and it was much more gentle on my ears.
I'm running an iPhone 13 Pro on iOS 16.1.2
This is absolutely unacceptable. It makes me furious every time it happens. Why hasn't this been fixed?!?!?!?!?!?!??
The ringtone volume is so loud in my airpods when I receive a call that Steve Jobs turns over in his grave.
