Disabled ITunes and App Store account

I had a subscription charge that I canceled because the app never worked. I submitted refund request but was told I had to file a dispute with my bank. I did but the refund kicked in so the claim was closed after. Now my ITunes and App Store says my account is disabled. How to I fix that? I tried Changing the password and entering a new card number and still says disabled.
