The purpose of the questionnaire is to see if your problem can be resolved without a visit, thus saving both you and Apple hours. Your other option is to use the Get Support link at the top of this page, explain to the Apple tech what the problem is, and if they can’t fix it they can help you set up a genius bar appointment.
