When I answer the phone my volume is always on low and I have to manually turn it up. Is there a way to change this so it’s on max volume when I answer my phone ??

While setting at maximum volume is really loud, you should set your desired volume in the Control Center. There is a slider where you can set your volume to your desired level. Here is an Apple Support document that will show you how: Access Control Center on your iPhone - Apple Support

If I turn on the background noise…. It plays the sound of water and birds. But when I call out or receive a call, I still have to manually turn up the volume in order to hear.

Your Volume slider is already at Maximum. I am wrong about your Ear symbol. The Speaker volume is at maximum. I sent you to the wrong symbol for your iPhone. I apologize. Mine shows with the Ear because I wear hearing aids.

I have accessed the control center but I do not see anything relating to volume.
There is one thing to check. Look in Setting, Sound &Haptics and see if Headphone Safety was turned on.
You may not have it added to your Control Center. Go to Settings, Control Center, Included Controls and see if "Hearing" is in the list. If not scroll down to "More Controls" and use the "+" key to add Hearing to your Control Center. It will add the slider with an "ear" in your Control Center.
Hearing is there. I have taken it off and put it back on. There is NO slider ….. see picture

If you Swipe Down from the Right Top Edge or top right edge of your screen and open the Control Center there should be an Ear symbol there to use the slider adjustment.
