What brand adapter?
Not all Mini DisplayPort to HDMI adapters will work.
What brand/model external display?
Try setting it to receive an HDMI signal for a computer.
Does the external display also have DVI or VGA?
If so, then test a Mini DisplayPort to either DVI or VGA adapter.

You should contact the nearest Apple Authorized Service Provider and have them check it out. Apple won't work on it as it's considered obsolete by Apple. The AASP may not either as parts may be hard or impossible to find.
It might be time for a new Mini.
I was hoping there would be some software based diagnostic option to determine whether or not the commands are reaching the engine room
