Hmm, check again that the lens is clean using a microfiber cloth:
Otherwise, try moving the phone closer or farther away from the QR code.

Ok thank you for your help. I will try the lenses look clean. When I put my camera closer to the object or QR code, It starts to be distorted. Is it any problem with the camera itself ?
