freeform is an app that is bundled with macOS. it cannot be deleted. and it only takes up 22 MB of disk space anyways.
but if it's on your dock and you want to remove it from there, you can drag it off of the dock and drop it on an empty spot on your desktop. it will then be gone from the dock.
Nope, it still added itself back to the dock after removing it again.
When right clicking on the icon, the menu has these:
OPTION - REMOVE FROM DOCK; OPEN AT LOGIN; SHOW IN FINDER
SHOW RECENT
OPEN
It's gone beyond annoying and becoming infuriating at this point.
Still dealing with this annoying thing of FreeForm adding itself back to my dock after a reboot.
But something I did notice, the only area in system settings that login items is, is listed in general. The users & groups does not have really any options to it (see screenshot).
SO here's an upate (no the problem is not resolved yet though).
I actually contacted Apple support and they had to go through all the suggestions already tried from this thread.
They also had me check to see it it was being synced via iCloud, and it was but turned it off but it still gets added to the dock.
They also had me make an account, and test it to see if the issue would be on another user, and the problem didn't happen with that other user account on the computer.
So, it's something about the user account that keeps adding it to the dock. Anybody have any other ideas of what could be adding it to the dock after each reboot?
Antivirus software: Apple, Symantec, and CleanMyMac
Completely uninstall symantec and cleanmymac. They are not providing you with any security, and instead may be conflicting with each other as well the built-in security.
In fact, cleanmymac is considered by many (me included) as malware itself. It is an aggressively peddled application, on the false premises that it will clean, optimize and secure your mac. It's a scam.
My guess is that you probably just dragged them to the trash, thinking that it would be enough to uninstall them. These things put pieces in different places so you have to follow their own instructions in order to properly uninstall.
For Symantec I had used CleanMyMac's uninstaller, for CleanMyMac I used AppCleaner.
But unless fully getting that stuff uninstalled would resolve the issue this thread is about, I will worry about that later.
Right now I am focused on how to make it so the FreeForm app stop being added to the dock after a reboot.
You can delete applications on a mac through the launchpad
See this guide for further instructions:

"Insult me again and I'll put an SSV on your computer."
Ooooh, oooh, can I have one?
It does NOT stop it from being added back to the dock after reboots, nothing seems to work. Next I will try deleting the dock preference as suggested by another post here.
I moved the location of the FreeForm, and after rebooting it did retain the location I moved it to on the dock.
Correct, I either choose "remove from dock" or drag it off the dock.
I have looked in the login items and it's not listed there. Is there any other area it would be listed to be adding to the dock?
It's utterly annoying that I must keep an app I don't use and I've never chosen to install.
When I remove it from the dock and reboot immediatly, it does indeed come back to the dock after the reboot, even in safe mode. ANy way of rebooting, it always reappears in the dock.
They both haven't been installed for months, not sure why the report has them listed.
