Someone keeps signing into my account I get notification change password this keeps happening I don’t know what else I need to do.

It’s take sometimes only three days and up to 5 weeks after I change password that I get notice from Apple that my account was signed into with a Apple 5s phone it never shows up on my account only my 14 pro and Apple Watch show up.
