Someone keeps signing into my account I get notification change password this keeps happening I don’t know what else I need to do.

Time is irrelevant as it always shows pst. As for the device, you can remove any associated devices at appleid.apple.com. And, review If you think your Apple ID has been compromised - Apple Support.
If the messages are sent shortly after you log in, it's most likely you, not someone else.

It’s take sometimes only three days and up to 5 weeks after I change password that I get notice from Apple that my account was signed into with a Apple 5s phone it never shows up on my account only my 14 pro and Apple Watch show up.
