I have issue with MacBook 30W adapter. I have checked cable - it’s fine, charging ports- it’s fine. The adapter is also connecting and then disconnecting after few seconds. What would be the reason ?

A few thoughts that may help...
A. Black Mac Screen:
Go Here: If your Mac doesn't Turn On or Start Up - Apple Support
Using External Devices: Try Resetting the SMC and NVRAM, but this time, using an external keyboard. Connect an external monitor and see if you get an output. This applies to Intel-Based Macs only.
B. Mac Power Issues:
Try a New Charger:
If it is currently charging from a charger, then use a new charger:

That’s the common answer, I have done all that. A new charger helps but the issue with old charger remains as it is. The charger is clean and like new. Less than 2 years old. Should I through it like e waste trash ?

Just Use the New Adapter:
Well, if a new adapter works, then there is obviously something wrong wit this one. It may be corroding inside, or it may have been damaged due to a power surge or power spur. So, take this to an eWaste recycling center (you'd likely find one at a local retailer).

What Risk a Damaged Mac? Seems to me this is a damaged power adapter --so use the new one. I'll leave it at that.
