My mid 2010 MacBook Pro has an average CPU temp of 69-70C when idling. Is this normal for a mac or is it time to change the thermal paste? I haven't had any thermal throttling yet but the paste will be quite old and crusty. How much of a difference will it make to temps?

Before change the thermal paste, clean up inside of your MacBook Pro, especially fans, fans are running normal?
Check the fans with Apple Diagnostics if it reports fan error.

You should get better thermal conductivity paste, a little bit expensive, though.
I have cleaned out the fans so I think it's time to change the paste
