Removing activation lock for person with brain injury

A relative with a memory-challenging brain issue has an iPhone 13. They forgot their password but still tried too many times to enter the password and now they are locked out. He needs his device to stay in touch with family. What can be done to reset the phone completely so it can be used again?

If the person knows their Apple ID, but can't turn off Activation Lock:

Start a support request

If you need help with removing Activation Lock and have proof of purchase documentation, you can start an Activation Lock support request.
