When I mute my iPhone in the cinema, an incoming call still rings in my hearing aids.
Can I prevent this, it is very disconcerting?

Seems you may want to try using "Do Not Disturb" on your iPhone:
Apple support have an article online (link below) to help with that:
How to: Use Do Not Disturb with Focus on your iPhone or iPad
When you have Do Not Disturb turned on, a crescent moon icon
appears in the status bar and on your Lock Screen.
(If needed: You can turn it off, in the same way you turn it on.
ie: By tapping the "Do Not Disturb focus" that you have set.)

PS: Another option is to use Silent mode:
In Ring mode, you hear ringtones and alerts.
In Silent mode, you don't, but your iPhone can still play sounds, like when you play music or videos.
