If you haven't yet, you can check permissions at Settings > Privacy > Microphone, and Camera, respectively. You may be able to grant or disable access to the app(s) here.

Regarding Safari, try these steps and test between each to see if anything here helps:
1. Connect to a different network
2. Restart your iPhone
3. Clear website data
4. Turn on JavaScript
5. Check for Private Relay
