Verizon PUK code for iPhone 12?

Verizon PUK code for my iPhone 12. Locked my sim up and can’t get ahold of a person. And can’t log into My Verizon app

Nobody here can provide a PUK code for you.

Only Verizon can give you that. You will need to contact Verizon in some way to get the PUK code or a different sim card, that is not locked.

In addition, if your enter the wrong PUK code too many times, it will require a new SIM card from your carrier. It is best to contact your carrier for the PUK code before that happens. They should be able to provide the PUK code over the phone, where the SIM card would require a visit.
