Have you had any repairs done on the phone? Who did them? If you had repairs done by anyone but Apple or an Authorized Apple Service Provider, you will see that notification.
If you have had no repairs done o the phone, it may indicate that something is wrong with your phone's hardware. Contact Apple to arrange for service:
